Thank you for your interest in being added to our Subcontractor Database.
Each of the following documents must be filled out completely and returned as soon as possible.
1) Sub Contractor Approval Checklist
2) Copy of current Insurance Certificate, with minimum acceptable limits of coverage as noted on the attached letter. Note:BCBE must be named as an additional insured once you begin working for BCBE. Payment for work completed will not be issued until BCBE is named as an additional insured.
3) Copy of current Insurance Policy.
4) Copy of your certified or registered Florida license including local occupational licenses.
5) Company profile, geographic information.
6) Provide three (3) letters of references on company letterhead. Topics to address include:
7) Form W9 - Request for Taxpayer I.D. No. and Certification.
These documents are required from all our bid list applicants and represent proof of qualifications. Your application cannot be processed without them. If your application is approved, a personal interview will be required with a representative of your company. Only after this interview has been completed can your company be added to our Subcontractor Database.
